Sounds like it could be valve seals - oil seeps past them when it's off, then when you start it up you get a cloud of burnt oil. Doesn't tend to be a problem when the car is running and everything is warm... Might explain the running on less than four cylinders during startup - oil may coat the plugs or otherwise inhibit proper combustion.
OR....
The hunting idle is usually the airflow meter - and it could cause symptoms like running on fewer than four cylinders while the mixture is way off. If the car is running really rich at startup, you could get a puff of smoke, but it'd smell like gas for sure... Try cleaning it with some CRC Mass Air Flow cleaner, or swapping it with a known good one. Also, be sure all your grounds are in good shape, and the battery ground cable isn't destroyed... bad grounds can cause wacky fuel injection issues...
