Can't add much to what's been said here. If this is a really nice car then it should be worth fitting a fresh transaxle. Whether you want to stay with an auto or swap to a 5 speed the best idea is to install a properly rebuilt unit, so you don't have to do it again soon and nobody has rebuilt more of these than Eriksson. Relatively few of these units go much more than 150,000 miles before they act up and fail. The lack of second gear suggests that the flex band is not applying due to either an apply circuit fault, like a defective servo, or just a really badly worn band. You can go through all of the motions, checking and re-fitting the governor, checking all the pipes and valves in the valve body, flushing and using additives, adjusting bands and cables but at 185,000 miles you are hoping for that very small likelihood that the unit isn't full of totally worn friction elements, worn rubber seals, worn thrust bearings and washers and circuit choking goo. Good luck with whatever you decide to do.
