I know it's a major PITA, but you might want to drop the knee bolster and a cover to verify that the heater core is getting up to temp. Reports of core faults are rare, but I know from personal experience that they happen. Specifically, I had low heat in my '88 and went through all elements of the system as you describe but to no good effect. I finally did the above and, for reasons I still don't fully understand, found that the driver's side of the core was toasty warm, the passenger half was ice cold. Since a fan forces air across all of the heater core, the "heat" provided to the cabin was an average of the two core temps-- i.e., not much.
FWIW, I just went out to check the heater hose config on my '93-- as you describe, hose from metal tube at top, the one to WP is on bottom and I have way more heat capacity than I can use.
