Rough shifting with dirty fluid sounds like dirty fluid to me. How can they diagnose an idle rattle as the transmission? Maybe they have an in-house psychic.
The one thing that concerned me from your post was what you said about the smell. If the fluid smells like the Dealership's hardest working mechanic's armpit after a 14 hour shift, you may have a problem. If the smell is really bad, I understand there is some possibility of hastening the demise of the transmission by changing the fluid. The new fluid will have detergent properties and can free up sludge to move around. Frankly, I don't know whether that's true on a car with 68K or not.
