I "never" have to replace calipers, but that is mostly because the cars I work on don't have very much rust problems.
If and when it happens (maybe once a year)I replace both of them.
I can always tell when a customer has been to "another" brake shop because it is usually new calipers on the car together with pads and rotors.
So there you have it, I don't upsell calipers on every brake job I do, but when I have to replace one I replace both of them to be sure I don't run into problems.
On the other hand, seeing what the calipers looked like in Sweden I probably would replace a lot more if I was working in that kind of enviroment.
