Since you feel the vibration at speed without the brakes on, I would suspect the front end alignment, worn bushings/ball joints, or both. If things are loose or misaligned up front, the front tires will wobble a little, causing the vibration. Under braking, the suspension geometry changes, and can make that vibration worse. I'd suspect that you might even find some scaloping on the outside edges of the tires.
The only exception here would be this case - When you first start driving, the car is fine. You use the front brakes a bit, and then the vibration is bad, even when not pushing the brakes. If you drive for a while not pressing the brakes (like 10 or 15 minutes), the vibration dies down. Use the brakes a bit, and the vibration comes back, even when you aren't on the brakes. In that particular scenario, you might have a sticky front caliper. When it sticks, it heats up the rotor and it warps out of true, causing a vibration. Even off the brakes, the warping is enough to hit the pad on every rotation, causing vibration at speed. If you stay off the brakes long enough, the rotor cools down a bit and returns to a less-warped shape. I've had this happen. But the vibration is VERY pronounced, and the pattern exactly as specified.
The other possibility is a tire out of true. I've had belts shift in a tire. You can balance it, but it still vibrates - after all, you can balance an egg, but I wouldn't want that shape as a tire. To check that, try swapping the tires front to rear - if the steering wheel stops vibrating, it's the tire.
But I'll bet an inspection of the front bushings will show tired, cracked rubber.
