The only time I change rotors is when I feel shuddering or chatter. Doesn't matter how nasty they look, scored, etc. There is a minumum thickness you can check, but for me, proof is in the braking.
I tried resurfacing and they lasted about 6 months. Resurfacing thins the rotors and with repeated heating and cooling, they warp vey quickly. Now I only replace rotors. If you use soft compound pads, which are usually the stickiest and provide more braking power, the rotors will last a very long time.
Don't know about ceramic pads.
