Make sure that the black plastic sleeve just aft of the bearing is pushed back to the base of the slave. This sleeve is sometimes not pushed back all the way and is sometimes 'forgotten'. This could also cause your problem. Once the sleeve is 'reset', depressing the clutch pedal will push your bearing back. You can use a screwdriver to push it back, a little bit on each side until it's back. Anyway, doing this kind of troubleshooting "online" is never easy. If all else fails, go back to the indy and explain the problem to him. He should know what to do.
Here's a summary of possible causes:
1. The black plastic sleeve is not pushed back.
2. The bearing is not fully seated on the slave.
3. The clutch plate binding/sticking on shaft.
