My suspicion lies with the heater core - I'm pretty sure I have the same problem. As Senor Don points out, a good test for the heater core is to measure the temperatures of the incoming and outgoing hoses - they shouldn't be too far apart (I don't think they're likely to be identical even with a perfectly clean heater core since the coolant will surely lose some heat as it passes through the heater core).
I had some good luck a couple of years ago flushing out the heater core. It still isn't operating how it should, but it's much better than it was. I disconnected the two hoses to the heater core and poured in some limescale remover through a funnel (I think I used CLR). I let it sit for a while and then flushed it out very well with a hose. Rinse and repeat.
I think this summer I will be replacing the heater core because on really cold days it's not up to the job.
On the issue of the blower being on high as soon as you turn the engine on, I would agree with you that doesn't seem right (my 95 waits until the air has warmed up before increasing blower speed) - except in my wife's 98 the blower does just hwat your does, so who knows?
