So just to "close the circle" on this...
In August, I got my hands on a new set of wheels, and a set of brand-new lug bolts.
Removed the old wheels, and realized there was still a lot of anti-seize on the mating surface between the wheels and the rotors. So I cleaned ALL the anti-seize off, and installed the new wheels with the new lug bolts.
Drove the car on a longish trip (~2 hours each way) and many shorter trips since and the lugs stayed tight (I checked multiple times) until I removed the wheels to install my snows in early December. So I'm going to call the problem fixed! My best guess is the main cause was the anti-seize on the mating surface I mentioned earlier - I think it allowed the wheel to rotate a little during acceleration and braking which eventually would loosen the lug bolts.
