The term "heel and toe" is a throwback from yesteryear. Now, it refers to braking with the ball of your foot/big toe, and blipping the gas for the downshift with the outside of your foot/little piggies. It's used for cornering while racing. Brake as you're in the turn, and you can downshift while youu're still braking. This way, you're in the proper gear for exiting the turn. The double-clutch part of it prevents the car from jerking when you're near the limit of tire grip (or the limit of your nerves).

It's kind of tough on Saabs, since the pedals are a bit far apart (at least on C900's and 99's). Keeping your heel all the way towards the center console, and just rolling your foot to blip the gas is the best way.
