vacume leak, poor fuel presure. But with O2 feedback the computer will do eveything it can to make the fuel mixture ideal. If you have a vacume leake the engine will just dump more fuel.
Try to use an analog multimeter and you'll see the voltage swing back and forth from near zero to 1. It might depend on where you get your ground. My readings were -.25 to .75 but you get the point. You're voltage average should be .5v - .95 lambda during idle. Your o2 sensor could be bad and reading rich which is making the computer cut back on fuel. So if this happens, the O2 sensor will actually read correct, but at the tailpipe you know its lean. I'm chaising a studder on slow acceleration and my O2 sensor output looked real good. The plugs look good too. So I think even if something is not quite right the engine is able to make up for it once it catches a problem starting to occur.
