As loud as you describe it I would be inclined to think the fuel dampener... Timing chains are loud when they fail but not so loud that most people stop driving their car (they drive past the point of failure). My fuel dampner would often (not always) sound as though the engine was comming apart, especially at idle. This sounds like the symptoms you describe.
The TSB (Technical Service Bulletin) was issued to Saab dealers about this problem and is posted on the Townsend Imports website (under engine then fuel)
The link for two pages on the fuel dampener are: http://216.78.172.244/engine_folder/newregfornoise.htm
http://216.78.172.244/engine_folder/pulsenoisefix.htm
The townsend site (click on the saabmaster link)
http://www.townsendimports.com
Sometimes the site is down so be patient and try back later if needed. The cost for the part if I remember correctly was about $250. Labor would be additional.
Hope this helps...
Scott
