If your car idles perfectly in neutral and it acts strangly in first or reverse then you should suspect the auto trans. Not sure how the trans is build, but normally it should load the engine evenly at low and high speeds. If there is a problem though the load to the engine is not even and at low speeds, rpm, it will try to stall the engine. The engine electronics will then try to compensate for the loss rpm resulting in the engine surging. This of course can also happen if the engine has less then normal power at low rpm, but then the idle in neutral is not likely to be right. Check color of spark plugs to see if the engine fueling is right. A lean or rich idle mixture will cause the engine to have small power at low rpm.
