1) your symptoms may be a fuel filter that needs replacement, more likely at that mileage, Dean may have more info on that, I do not recall FPR's going bad on the V6 (fuel filter is an easy job, use jack stands if you DIY!) there are instructions posted here, either me or someone else did.
2) I don't recall, but with them usually what's in the picture will show up in the mail, look at the schematic they show on the left hand side if there is a separate part # for o-ring.
3) pretty easy, it's just a few turns of a wrench. Search for my detailed instructions on the process. Do it on a cold engine b/c there will be a small amount of gas escaping. Replace the small vacuum hose while you're in there, its' a thin hose, and costs likely less than a buck. ( I replaced mine trying to fix a hard start/idle problem, when I pulled it apart it was still tough as nails, and no evidence of a leak.
