Engine flush machine only works on engines that doesn't need it.
My old boss has a Bilstein one and it is a joke.
It will not remove caked up sludge from the valve train or timing chain area.
It only pushes solvent through the oil galleys and out through the oil pickup screen.
It "might" remove some carbon particles from the oil pick up screen but that's about it.
If the engine is already sludged up you need to get the valve cover off and drop the oil pan and manually remove the sludge with brushes and brake cleaners.
As a first measurement I would open up the oil filter and look for metal debris and also remove the oil pressure valve to look for scoring. If the pressure valve looks bad you probably have worn out balance shaft sprockets and in that case, the front engine cover needs to come too.
