The timing chain tensioner is screwed into the the side of the head, close down by the head gasket, on the back of the head. The inner bit with the spring screw out first, then the body. It can be removed and inspected. When the oil pressure gets up, that adds to the spring force. The thing slides forward in a ratchet that can be held in to retract it. After the main body is fitted, the center bit with the spring is inserted and tighened down.
It should be possible to detect if anything is wrong with it. It pushes on a swinging chain guide. With the tensioner removed, you should be able to poke something in and push on the tensioner and feel it move as slack is taken up. I don't see any problems in doing this. Just don't turn the engine or run it with the tensioner removed.
The head cam cover can be removed and the chain rollers can be inspected for grooving, and the cam shaft sprockets can be inspected for wear. The driving side of the teeth can become cupped, just compare to the other side.
I have not looked to see if the tensioner is buried under other stuff that makes getting at it a pain.
So an inspection could be reasonable DIY. The chain and sprockets for sure, and the tensioner depends on easy access behind the RH rear of the head.
