The chain tensioner is hydraulic, it does get 'pumped up' by the oil pressure. Also, it's accessible externally, you can pull it out and see how far it's advanced. (It's not quite *that* simple, but you don't need to take the cover off.)
I don't think they normally fail, but the amount they've extended seems to be a good indicator of how much the chain's stretched. I think the max is 17-18mm, and the point where Saab says it's too much is 11mm.
Of course, anything else like chain guides and sprockets require you get the cover off.
