I live in Chicago where just about every frequency is used and have done quite a bit of driving around the midwest lately ~20k in 6 months. I've never had interference with my XM SkyFi and the Delphi FM modulator. When the XM's on all the FM stations come in fuzzy so I assume that it does some signal blocking when activated. When the XM's off the FM stations come in clearly, but I never listen to FM anymore.
