A smarthost is nothing more than a mail relay... unless you know of another mail server out there on the internet that will accept and relay your mail (unlikely) this isn't gonna get you anywhere.

Have you verified that 1and1's mailserver has been blacklisted? Is it with one of the reputable BL operators or something of the remote host's own design? If it's 1and1, and a reputable BL, call 1and1 and tell them to fix it - you're paying for their mail service, they should by all means get your mail where you sent it. (I work for a small ISP; we get blacklisted from time to time due to customers with viruses, etc., and I get on the phone and call the folks doing the blocking and they invariably unblock us quickly)

I would personally NOT put up an exchange server to handle this. In the first, maintaining and securing any mailserver much less exchange is no small task, and in the second, you may find that many large mailhosts out there are now blocking ranges of IPs that are known to belong to cable/DSL/wireless pools without regard to who's actually on them. You may find an exchange server located on a DSL line may be blocked in exactly the same way.

If 1and1 isn't fixing the problem, go find a new host. FWIW, I've used godaddy for years, who outsource their email to secureserver.net, and not once have I had any issues with their service... it seems to be rock-solid. Dealing with secureserver people as an outside mail relay is pure hell (they blacklisted my company once... oopsie!) but their email service is great.
