It is considered rude - but it still happens. Especially the French and drivers of black Golfs (not to mention French drivers of black Golfs...) tend to do it.
I've only ever done it two or three times in order to avoid an accident - like when driving fast on a near-empty Autobahn (130-140 mph) and someone ahead pulls into the left lane to overtake someone at a relatively low speed. I can tell you that people get out of the way in a jiff when they see those headlamps rapidly getting bigger!
If someone stays in the left lane for some time without any reason the polite way of waking them up is to turn on the left indicators.
