I have the same issue, and still trying to find a solution. My (untested) theory is that the increased cross section of the pipe is lowering the density of oxygen. (Note that while it doesn't go to 3" until after the sensor, it is definitely larger than stock before the sensor.) What the theory doesn't explain is why this is only happening to me (and now seemingly you as well, though our problems could have different causes).
Jak suggested I try moving the sensor further downstream to a straighter part of the pipe, which I am going to try since it's simpler than my idea of re-making the first bend of the pipe in stock diameter.
(p.s. I have tried three different o2 sensors, checked wire continuity, and measured the voltage signal at the ECU)
