mid-range?

so are we at an optimal(OEM) set-up for mid-range? A small turbo like the Mitsu spools faster, but then the exh. manifold pressure increases? Better or worse for max. fast/steady mid-range?
All my driving is in a western city (i.e. spread out) where 5th gear is used infrequently.
Everyone talks about boosting max hp/torque at max. rpm. How about us meek mid-range guys. What can we do to help the air pump be a little more efficient mid-range?
Ext. waste gate? 3" down pipe? Unequal header to turbo? Mitsu vs T3? Diff. head? 9000IC? water inj?
Be gentle, I'm just an mech.eng. with limited air pump experience!
