You shouldn't be required to pay Enterprise the $18 (or whatever amount they want) for insurance. They tell all customers (even those who are paying themselves) that the customer will be responsible for all charges if there is any damage to the car if the customer doesn't pay the surcharge.
But, the insurance you already have on your own car may quite possibly cover the rental/loaner car, provided the rental car is replacing your own car while it is in the shop for service. Check with your insurance company to make sure. In fact, many insurance policies cover rentals even if the rental is not a replacement when your car is in the shop.
Also, if you are renting a car (not a free loaner) and pay for it with your credit card, many credit cards provide you with insurance coverage on the rental car.
Don't pay rental companies money for insurance if you don't need to! Think of it: $18/day equals a premium of $6,570 per year, and this is only for collision and comprehensive insurance, not liability! Outrageous!
