If a car is tuned to run on 87 or 89, then using fuel of a higher octane is a waste of money.
If a car is tuned to run on premium fuel AND has knock-sensing capabilities (such as Saab turbo's), then using fuel of a lower octane will result in higher mpg's since the ECU will be retarding/advancing (again, I forget which) spark to avoid damage to the engine. A car cannot "take advantage" of running on premium fuel if that car is designed/tuned for premium fuel. The engine will not give any more boost than pre-set limits nor will it make better use of spark timing than what the ECU is set to allow, all-things-stock.
Now, if a person consistently drives conservatively (no jack-rabbit starts, max speed of 55 mph) and keeps the engine and tires in tip-top shape, then yes, that person will be able to achieve higher mpg's than someone with different driving personalities.
