P1135 and P0125 & broken vacuum hose

2004 95 Aero Wagon, CEL came on and Autozone reported the codes as P1135 and P0125. From research here, it looks like P1135 is "O2S Heater Circuit. Current too Low, Sensor 1" and P0125 relates to low coolant.
While checking the wiring for the O2 sensors, I happened to find a broken vacuum hose from the throttle body going down to a T nipple, which I replaced. Hoping this was the cause of the CEL, I disconnected the battery to clear it, but it came back again after a few restarts.
So a couple of questions:
P1135: Is sensor 1 the upper sensor or the lower sensor? Would you just replace this or would you recommend checking anything else?
P0125: My coolant level was fine, I topped it off a little bit more to get it to the upper line, but what else could have caused that code? There are no coolant leaks in the system or on my driveway that I can see.
