Well, this happened on an 01 V6 when the person that did the timing belt, did it wrong and long story short, the belt skipped 2 cogs at idle. 1 cog slip is ok. 2 or more not good. They ended up having to repair one head, redo the idle belt, etc and engine has been good since. It took 11k miles for the belt to slip.
No real way to know the damage until the heads are pulled and the pistons are inspected. If at full speed and the belt fails, can take pistons with it, etc.
If you are interested in selling the DICs, please let me know.
You can look in the intake / exhaust manifolds and see if the valve are bent, usually.
