It could be that the clutch isn't disengaging fully, either because the master or slave cylinder is faulty, or because the clutch shaft bearing is shot.
If either of these problems exist, the transmission will usually grind going into reverse and may be difficult to get into 1st.
Of course, it could also be a problem with the shift mechanism in the console, a motor mount, or perhaps something else.
The worst something else is that the pinion bearing is shot because this means $$$$. If the pinion bearing is bad, you'll hear a whine from the tranny in 3/4/5 above ~30 mph, particularly when accelerating.
