Motor mounts die. And the older they get, the more motion they allow, and the more motion, the more ripped up. As the lowers age, the bushing in the upper mount (dogbone) tear. I'll bet that the bushings in the upper dogbone (torque rod) are also shot. Which came first? Probably all a race to the bottom, but very often the uppers go, making the lowers move more. The mounts in your car are now 18 years old. Any car component that can register to vote and moves is probably at least 5 years overdue for replacement.
And I've never seen one good mount and one bad mount. It's always at least one bad mount, and the others are borderline at best.
I'll assume that you need to replace the lower two mounts and both bushings in the upper mount. Chances are the lower tranny mount should be replaced. I would also check all the other suspension components. The engine sits on a subframe, connected to the body by rubber. Check at the rear of the lower A-arm where it meets the body - that rubber bushing is probably in poor shape, also. It's not hard to replace.
