Brian,
You need make sure that the black plastic tube that slides over the metal shift shaft is oriented properly. At the bottom is a molded "foot" that should be oriented toward the passenger side of the car. When you go into fifth gear that foot rides against the white plastic base that you show in your picture. Lifting up on the black plastic collar under the knob lifts that "foot" and allows you to move the shaft further over toward the passenger side to access reverse (a backward motion as you know). I replaced a knob last year without glue or anything and orienting that foot was the key to normal operation. I hope this helps.
