What's up with my rear control arm bushing?

Haven't posted in a while--been busy with work and 2 little girls, but we still have 3 9000's (all with 200k+ miles)!
I'm working on replacing the rear control arm bushings on the 91 9000S.
I have the control arm out, and was able to remove the old bushing ... mostly. The metal tube that "supposedly" slides over the threaded rod is still in place with chunks of rubber attached to it. However, when I looked closer, it appears to be part of a larger metal piece. This larger metal piece has a big washer, the metal sleeve of the bushing, and part of the control arm.
How the heck do I get the new rubber bushing on? It already has the metal sleeve bonded to the rubber. I'd have to cut the bushing to shreds to use the metal sleeve that appears to be WELDED TO THE control arm!
Am I crazy, or just missing something obvious. It almost looks like I need to replace the control arm! Or just cut the whole thing off and have someone weld on a threaded rod of the correct diameter.
I'll see if I can post pictures later, but I could find no mention of anything like this in the archives except for "Roman" stating that 88 and prior models had the rear control arm bushing fused to the control arm.
