one on each end of the bushing, see picture of the front bushing below. Both ends of the bushing take identical parts that slide over the end; the mounting bolt goes through the hole in the bushing, through the hole in the bushing caps, and then through the subframe. If I recall correctly, you will need two 5/8" or 16mm combination wrenches, as one or two of the bolts have clearance issues and a socket wrench won't fit.
These parts are readily available aftermarket from a couple of suppliers. I just bought a pair from autohausaz, along with a bunch of other suspension parts - the potholed roads here in Chicago are pretty tough on suspensions.
