Source of vibration -found!

I have been experiencing quite a bit of vibration in my 88 9KT, which I have been blaming on my switch to solid type motor mounts shortly after I had my tranny replaced. This weekend, whilst replacing a busted heater hose, I noticed that the top front bell housing to engine bolt was backed out 5 or six turns. Investigating a little more revealed a stud at the top back of the bell housing with no nut on it. I tightened the bolt and the vibration is basically gone.
Question: Should there be a stud in the location I described? Or should all the fasteners for the bell-housing be bolts? If there is a stud in this location, should it have a lock-washer and nut?
Obviously the shop that did the tranny work did not quite finish the job. I have driven around for about six months like this. Is any damage to the tranny likely?
