Your car is a drive-by-wire concept where a computer tells the throttle to open up instead of you pulling a cable when pressing the accelerator.
Inside the throttle body is two potentiometers that the fuel computer is using to determine where the throttle plate is and where it should be.
The carbon traces that deliver the signals through two metal fingers running over the traces was not that good of quality and sometimes a glitch is created that the fuel computer see as a problem and goes into limphome mode.
One of the early software upgrades to the fuel computer was to widen the parameters for what the computer sees and accept as being good so that it wouldn't trigger limp home mode too often.
The first throttle bodies also seems to be not that good of quality.
Volvo have had the same problem with theirs from 99 and up to 2001.
But it was made much worse on the Volvos since they don't have a backup limp home throttle cable that could at least help you get home or to the shop.
I would first of all make sure that the throttle body is cleaned out (if not tried already) and after that, check for that you car have the latest software loaded in the Trionic computer.
After that, you still might have to replace the electronic throttle body.
