Could be something as simple as the spark plugs. Check the plug gap. Faulty or plugs with too wide a gap can break down under load. There is less load on the engine in first gear than in 5th at highway speed. I believe Saab calls for new plugs every 25,000. If you have the platinum plugs you should check the gap every 25,000 if you plan to run them for 100,000.
