DI Cassette nightmares continue. Plz Help

1995 900se 2.o Turbo conv 81,000 miles
The car just stalled on the freeway and was towed home. I crank it and can smell unburned gas fumes being puked out of the exhaust (at least there is fuel delivery).
Just changed the original Direct ignition cassette (DIc)10 months ago with 70,000 miles on it.
Two days ago the car idled at 2000 rpm for a while and settled down later.
I pulled the cassette off the plugs and had it cranked while I stuck a voltage sensing screwdriver in the DI's plug cavities (one by one).
Number 4 came on like xmas tree. 1,2 and 3 barely had the screwdriver bulb burning. All indications are a bad DI cassette.
The only warning I had was when two days ago the car idled at 2000 rpm for a while and settled down later. With the original, the car idled erratically for a full month.
Here are the questions.
1. With the DIc in place , when I crank the car at times seem to want to come to life and sputters. Is this because only #4 is firing properly?
2. Is there anything else that can cause the DIc to behave this way (only #4 firing) other than the cassette itself.
3. The bolts on the cassette were loose and could be hand-opened. Could a not-fully tightened DI cassette fail prematurely at only 10,000 miles. Remember that the original lasted 70,000 miles.
