What I do when getting a car ready for paint is > if it's easier to remove a part than mask it, then it comes off. You could always remove those parts yourself before taking it to the shop, I mean I would if it was my car. Makes for a much cleaner job IMO.
If the original paint is still in good shape > no pealing or other weird stuff going on, then it is assumed to be a good base to put more paint on top of. The shop would just fix the areas that needed feather-edged or filled, then prime, especially if they hit bare metal. The rest of the car could either be sanded with a minimun of 400 grit, or scuffed with a scotch brite pad. That's how we do things where I work.
$5000 paint jobs are not out of the question, but you're talking about much more work involved, or price of materials...custom colors, stripes, graphics, color changes. Paint is expensive! Some colors cost hundreds of dollars per pint....and remember this, when one is paying for a show quality paint job, that's what they want > perfection. Do it over if it's not right the first time.
