Although the disorders he mentions are rare, new-onset asymmetric shoe wear requires an evaluation. Typically, we don't divulge the complete "differential diagnosis," or the list of all possibilities, as this would cause a great number of people to lose sleep needlessly. However, the doctor needs to be thinking about these things. The physical exam is by far the most informative piece of the puzzle, and in most cases like this, expensive tests are not required. As far as expensive orthoses, which someone mentioned, I frequently recommend $30 over the counter brands, many of which are better than some of the products that are custom made. But I digress...

Primary care physicians typically don't have the training to address these issues, but some do. Orthopaedic surgeons, sports medicine physicians, neurologists, and physical medicine and rehabilitation (PM&R) typically do.

If CmacD has been experiencing this type of asymmetric shoe wear his entire life, then I wouldn't be so concerned.
