If you avoid them like the plague, how would you know what most of them are like? It certainly hasn't been my experience. If this is really eating you up, then you should look into the inner workings of a doctor's office and find out why they end up behind schedule. Not to fix it (impossible anyway), but to understand it. And going over some hypotheticals helps. If a patient in front of you has an unexpectedly complicated case, or if their procedure becomes tricky for some reason, what is the doctor to do?
One thing I can tell you is that doctors' schedules have become increasingly crowded. Except for private offices, scheduling is completely out of the doctor's control. Some organizations will set the density of a schedule at a certain level, hoping that everything will even out. There will be a few difficult cases, but then some easy ones. Some days, though, just like with all other areas of life, nothing is easy. Other places maintain an unrealistic schedule, however, and these are the ones that are chronically behind. Guess what type of place ends up having high staff turnover?
As far as saving time by doing your own car repairs, I agree that it usually doesn't work. I do my own repairs to save money and to make sure I know what's going on with my car. If I screw it up, I know exactly how, but if a shop screws it up, I may not find out until much later.
