If you drove 250 miles on 12 gallons that's 20.8 mpg. I am betting your heavy traffic morning commute is what kills it. Try clearing your SID (hold the clear button until it beeps) and do your morning commute. See what it returns. Then clear it again before your evening commute. I bet you're going to get around 15-16 for the morning, and 23-24 in the evening, guessing what your commute is like.
The SID on my '04 9-5 (which I trust - I've calculated it out and it's very accurate) stays around 18.9 most of the time. I don't do a lot of highway driving. Just some back roads and "city" driving around UConn's campus, and I tend to drive with spirit. If I clear it and get on the highway it jumps up to around 27-29. On long trips using cruise control I can get just above 30.
Regarding your MAF though, if there's ANY question that the thing is lazy, you should do something about it ASAP. It probably would help your MPG's too. A malfunctioning MAF can cause serious damage.
