The only path for water to get into the exhaust would be through a cylinder, since it would have to be pushed by a piston through the exhaust valve into the manifold. The coolant would have to leak into the cylinder through either a crack in the block (highly unlikely), through a crack in the head (more likely), through a passage of coolant into the oil supply (possible), or through a faulty head gasket (much more likely).
I'd stay start at the beginning again. Let it sit overnight, then remove all the spark plugs in the morning. See if any are wet with coolant. Check the oil dipstick to see if there is any coolant swimming at the top. Then, have someone crank the starter while you shine a shop light on the holes to see if any coolant is being spit out. That would identify a cylinder with a defect.
