My trusty 82T with 200+K miles still passes the CA smog test with everything spec'd and replaced just before the test - oil, filters, distributor, O2 sensor, plugs, no vac leaks, high octane fuel, spec timing. And it even has an EGR valve. It passes pretty much right down the middle of the scale. The last time I had it tested, however, it kept failing with high NOX. After several retests, the quite good technician then discovered that one of the spark plug wires was loose at the plug (No. 4 to be exact). Loose meaning the lead was close to or touching the spark plug head, but not snapped in place. It was snug enough to run the engine, but loose enough at high RPM's to cause mistiming or weakened spark. With the wire back in place, the car passed within the limits. Point being something that minor can cause the engine to fail.
