The threaded rod inside the piston assembly was some designer's idea of a brake adjustment mechanism. It's got very coarse threads though, so doesn't take that many turns to get all the way down, it's just very hard to get too if the caliper is on the car. I've used bent needle nose pliers for years, but have torn up my knuckles plenty doing it that way!
I'm just too lazy or cheap to make or buy a special tool...
It probably doesn't need to be all the way in, unless you have brand new brake pads and nominal rotor thickness. But get the dust covers on securely, since you don't want dirt or grime (salt) on the piston or in the cylinder. that will cause rust and caliper failure.
