The pilot bearing in my '89 SPG had a bent ball cage with little bits of shrapnel locking up in the bearing races: this was at about 110k miles. It made a heck of a noise when I was shifting, not to mention making it really hard to get it into 1st and reverse :-)
I've seen pilot bearings in cars that I've just pulled the clutches from in the pick-n-pulls that can't be turned by hand. These have been fresh cars with flywheels that haven't been exposed to the elements yet. That's not to say that a lot of them don't last well into the 200k mile range and beyond.
You're absolutely right about wearing the clevis pin hole in the clutch pedal assembly. That condition will show up as a low disengagement point.
