Another possibility though I'm not sure it's very common is that the clutch disk itself might have busted up, so that its center is broken away from the outermost part with the friction surfaces.
It's linked by six flexible segments of sheet metal, pretty tiny though they're of hardened quality steel. With some of the cheaper replacement disks out there, or many miles plus some beating on a good quality one, they could break.
This happened once on the original disk on a VW Rabbit I had, at 100,000-plus miles. Then shortly after I replaced it with the Brazilian one the auto parts guy sold me, the center hub forging of that busted up and I had to go back in again.
It made a kind of funny ring-tingling as the engine idled, with the busted ends I guess touching randomly.
