The coupler connects two rods; the rod that comes out of the transmission and the rod that projects forward from the shift lever. The coupler is pinned to the first and clamped onto the second and for proper transmission control that clamp should be loosened, the two rods carefully aligned and then the clamp tightened. Aligning the rods is done by placing the transmission in third gear and inserting pins (drill bits of the right sizes will work) through special holes in each rod before tightening the clamp and removing the alignment pins. That process synchronizes the shift lever with the transmission internal shifting mechanism. It is entirely possible to tighten the clamp with the rods so far out of alignment that gears at the left or right or forward or backward extremes can't be selected at all. Your situation sounds like the clamp was slightly loose and the relationship of the rods slowly shifted until you now have such an extreme condition that it's preventing selection of the leftmost ranges (first and second). It could also be due to or exacerbated by a bad motor mount and/or a defective coupling among other things but it would be wise to check that alignment.
