Not a Saab, but a general car question...

Got a call late last night from my mom-in-law. She was stuck at a gas station because her '95 Honda Civic EX blew a spark plug right out of the engine on her way home from work. Unfortunately, I could do nothing but speak with her and the station attendant since it was too far away for me to get there.
My question is, is this something that happens? I have worked on cars for many years, and I have never had this happen to me, let alone ever hear of it happening. I do all the work on her car, and the last time I touched the plugs was about 3 years ago. She doesn't drive much, so the intervals are far and few between for changing them.
Can this happen over time? The engine has an aluminum head, if it matters, and has 72K on it. Overall, the car is in perfect condition, as I maintain it regularly. I know the plugs were tight when I installed them, and since it was 3 years ago, it wasn't from my changing them.
I'm just looking for some general engine opinions on this one. It looks like she might become a fellow Saaber after this if it will cost too much to fix it. I have a '96 900S for sale, and she might be getting it now!
Thanks!
Joe
