The caliper pins were well greased.
I swapped the rotors and pads from side to side last evening. The noise is now barely perceptible on the RH side (previously it was loud on the LH side). I'm not sure if the change in severity is due to the swap from side to side or the fact that the surface of the rotor has been worn down during this troubleshooting exercise.
I suspect the cause of the problem was the ridges from the machining operation as SWEDECAR theorized ("playing the record").
