Since I've now replaced the serp belt twice, I can say that I never needed the use of some special tool on that tensioner pulley. Even the first time, when the car had 70K miles on it, the bolt was removable, albeit with some extra persuasion. This was also done without the help of another person.

When you are removing the pulley bolt, you must turn the bolt clockwise or to the right (opposite to the norm). By doing that, you are pushing up on the handle of the ratchet or towards the rear of the car. This effectively pushes against the tensioner. The tensioner should be at its max height at this point since the belt is no longer pressing against it (belt should have been removed from the pulley already). Actually, what you are doing is using the bolts that hold the tensioner to the engine as the resistance factor. Hopefully these bolts are strong enough not to break when applying the extra pressure against them as you are trying to push up on the pulley bolt to remove it.

I've used my foot on the end of the ratchet handle, and gave it a swift kick. That's always been enough to get that pulley bolt started to move.

Again, maybe for those bolts that just won't budge due to whatever reason, the tool may be of use, but for the most part, it might be an unnecessary expense.
