Top radiator hose coming off?

Twice now in the past 2 weeks, the top radiator hose has come off while driving. The first time, I was lucky enough to be right in front of a gas station, and lucky enough to have my multi-tool in the car. The second time (today), I still had the spare mixed coolant in the car.
I obviously re-tightened the hose clamp, but I don't want to crank it down too far and crack the radiator outlet.
Is there any other reason for the radiator hose to be popping off? There's no cracking that I can see; it's just coming off. Could the expansion tank cap not be venting correctly? Any other ideas?
